Presentation
A previously healthy college student developed a slowly worsening fever and confusion. A transesophageal echocardiogram (TEE) revealed a mitral valve vegetation; brain MRI imaging showed several small strokes suggestive of embolic spread. Blood cultures collected at the time of admission became positive after 3 days of incubation with a short gram-negative bacillus.
