Presentation
The patient is a 53-year-old male with multiple myeloma diagnosed in Aug. 2023. He was placed on a chemotherapy regimen for 7 months following diagnosis. He presented to the Memorial Sloan Kettering Cancer Center in May 2024 for a planned autologous stem cell transplantation. The patient developed non-neutropenic fever, cough and diarrhea, which revealed parainfluenza virus in a nucleic acid amplification test. A positive blood culture from this patient grew 4 gram-negative organisms.

Additional Answer Information
Identification using the Cobas eplex BCID panel and the BioMerieux MALDI-TOF system revealed Klebsiella pneumoniae, Enterobacter cloacae, Stenotrophomonas maltophilia and Acinetobacter nosocomialis. Antimicrobial susceptibility testing was performed using the MicroScan WalkAway plus System (Beckman Coulter, U.S.) and these bacteria were susceptible to all antibiotics tested in the panel. The patient was treated with sulfamethoxazole and trimethoprim (Bactrim) and Meropenem.
